Student Registration

Children attending Salmon River Schools must be district residents, and all new students must register with the district registrar.

Registering a New Student

To register a new student, parents can create an account by visiting ClevrCloud and selecting “Request Access Here”.

Learn how to create a Clevr Parent Account and how to navigate our online registration tool.

After creating an account, parents will have the opportunity to complete the virtual enrollment form and upload any of the following materials:

  • Birth Certificate
  • Last Report Card (if applicable)
  • Immunization Record*
  • IEP For Students With Special Needs
  • Any court papers (should there be any)
  • Proof of Residency, such as:
    • Pay Stub
    • Income Tax Form
    • Deed or Lease (to house or apartment)
    • Utility bill or other bills (sent to the student’s home address)
    • Membership documents – such as library cards – based upon residency
    • Voter Registration Document
    • Official Driver’s License, Learner’s Permit or Non-Driver I.D.
    • State (or other government-issued) ID

*NYS Law Requires parents to have a copy of child’s immunization record and proof of residency.

Student Transfer Notice

The Salmon River Central School District’s transfer policy deadline is June 1.

This policy applies to all students who wish to transfer into the Salmon River Central School District (including the St. Regis Mohawk School) who are changing school districts but not residency.

Enrollment packets are available in the Office of Instruction, located in the Elementary of the Salmon River Central campus or at the St. Regis Mohawk School. The student’s birth certificate and immunization record are required at the time of enrollment.

Please be aware that the following New York state requirements are necessary for entry:

  • Grades 6-10
    • (1) Tdap vaccine
    • (2) Varicella
    • (4) Polio vaccines
  • Grades 7-11
    • (1) Meningococcal vaccine
  • Grade 12
    • (2) Meningococcal vaccines

Please check with your healthcare provider for New York State requirements of your child’s vaccination schedule.

Pre K & Kindergarten

Health Questions

The Pre K & Kindergarten Health Questions Form is required to be filled out for all new students entering grades PreK or Kindergarten.

Lead Screening

Students entering grades PreK or Kindergarten: The Lead screening form is only required for students entering PreK.
